Special Concerns Response Information

 Seminole County E-911 maintains a confidential database of residents with special needs, including (but not limited to) individuals with Autism Spectrum Disorder, dementia, Alzheimer's disease, intellectual or developmental disabilities, and other medical or cognitive conditions that may affect emergency interactions.

Information provided on this form will be entered into the Computer Aided Dispatch (CAD) system and linked to the individual's address. This information is only accessible to emergency responders during an emergency to help ensure a safe and appropriate response.

Please complete this form as thoroughly as possible and return it to Seminole County E-911. 

A recent photograph of the individual is also requested.
